Form 1095-C Availability Notice
In accordance with the Paperwork Burden Reduction Act and applicable Internal Revenue Service guidance, The Turman Group and its member companies will file Form 1095-C with the Internal Revenue Service for the applicable tax year but will not automatically furnish copies of Form 1095-C to employees.
What is a Form 1095-C? Form 1095-C is an informational document detailing the health insurance coverage offered by your employer. For most taxpayers, it serves strictly as a personal record and is not required to file a federal tax return. The form is primarily used for tax preparation if you are claiming the Premium Tax Credit for Marketplace insurance or if you reside in a state with its own individual health insurance mandate.
- A copy of your Form 1095-C is available upon request.
- Current or former employees of The Turman Group who wish to receive a copy of their Form 1095-C may request one by contacting the payroll representative at their respective Turman Group location.
- Requests should include the employee's name and preferred delivery method (mail or email).
- Requested Forms 1095-C will be furnished within 30 days of the request.
